Position Description

Title: RN-LTC

Location: Lakefield, MN

Shift: 8 Hour Eve/Noc mix, 40-hour GT

Length: 13 Weeks

Weekly Gross: $2200 - $2400

Job Summary

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our team at a Long-Term Care (LTC) facility. The ideal candidate will have strong clinical skills, a deep understanding of elderly patient care, and a commitment to providing high-quality, compassionate care to residents. As an RN in a LTC setting, you will provide essential nursing services to long-term residents, ensuring that everyone receives the personalized care they need. Your role will involve direct patient interaction, coordination of care, and ensuring compliance with all healthcare standards and regulations.

Duties:

  • Provide direct nursing care to residents, including monitoring vital signs, administering medications, and assisting with daily living activities.
  • Develop, implement, and evaluate individualized care plans for residents in collaboration with the healthcare team.
  • Ensure proper documentation of nursing assessments, patient care, and medical interventions in compliance with facility protocols and HIPAA regulations.
  • Assist with wound care, catheter care, and other specialized nursing interventions as needed.
  • Monitor and assess residents’ health conditions and respond to changes in status, reporting findings to physicians and families as appropriate.
  • Coordinate with physicians, therapists, and other healthcare professionals to ensure the highest level of care for residents.
  • Provide support to residents and families, addressing any concerns and facilitating communication.
  • Ensure that care provided is in line with current best practices, nursing standards, and regulatory requirements.
  • Assist in training and mentoring nursing staff and CNAs (Certified Nursing Assistants) in proper care procedures and facility policies.
  • Promote and maintain a safe, clean, and comfortable environment for residents and staff.
  • Provide support during emergencies and assist with crisis intervention as necessary.

Requirements:

  • Valid RN License.
  • Graduated from an accredited school of nursing.
  • Current BLS-AHA certification.
